Clinic Opening:
As with any new business, there was a lot of ground work that needed to be covered before the Grand Opening. Because of a non-complete clause in our contract with our former employer, we decided it would be best to temporarily move our practice just outside of the 15 mile non-compete radius. We have moved into 6363 West 120th St., Suite 202 in Broomfield.
Your Medical Record:
There have been many questions regarding request and transfer of your medical record. We encourage you to request from Longmont Clinic / Carbon Valley Medical Center that your medical record be sent to our new address before your visit with us. You may download a request for records form at our
www.NorthVistaMedicalGroup.org website. You should not be charged (by Longmont Clinic or Carbon Valley Medical Center) for your record if you request that it is sent to our new office. If you are charged please let us know.
Insurance:
There have been a few patients that have received a notice from their insurance company stating that we (Dr. Tusek and Dr. Flanagan) are no longer providers under their plan. This was temporary. When physicians leave employers they must reapply to the insurance companies. We have applied to all of these companies and currently are accepting most, however, some have yet to finalize paperwork on their end. We plan to accept all of the insurances we did previously, as well as, numerous others.
Tri-Town Medical Center:
With challenges come opportunities and this transition has been filled with energy and excitement about our future plans. We have had many discussions with town leaders, other health care professionals, our patients, and many throughout our community. We are currently in the process of securing land within the Tri-town area and once that happens you will see our new medical center being built.
During this process we will continue seeing our patients in our temporary location 15 miles south of the Tri-Towns, but once our non-compete expires, our new permanent facility will open and we will return to our community!

The tremendous amount of support we've received from everyone has been wonderful and encouraging. As we've always said, we are committed to providing caring health services to our community. Although we have to relocate temporarily, we will be back soon. We sincerely appreciate your understanding during this transition.
