Your Medical Record:

There have been many questions regarding request and transfer of your medical record.  We encourage you to request from yor previous physician's office that your medical record be sent to our new address before your visit with us.  You may request a records form by calling our office at 303-501-2600.  You should not be charged for your record transfer if you request that it is sent to our new office.  If you are charged please let us know.

Insurance:

There have been a few patients that have received a notice from their insurance company stating that we (Dr. Tusek and Dr. Flanagan) are no longer providers under their plan.  This was temporary.  When physicians leave employers they must reapply to the insurance companies.  We should be on all the insurance plans we were on previously.  If you have any coverage issues with your insurance company, please let us know. 

You the Patient:

The tremendous amount of support we've received from everyone has been wonderful and encouraging.  We are excited to be back in the tri-towns.  As we've always said, we are committed to providing caring health services to our community.  We sincerely appreciate your understanding during this transition.
